Terms and conditions for 10 Class Pack

%%INITIAL%%MONTHLY MEMBERSHIP AGREEMENT
This contract is made between you and Fit4Mom Irvine Tustin and is a month to month agreement and allows you to attend classes within the Irvine/Tustin franchise.


AUTHORIZATION FOR PAYMENT
You authorize Fit4Mom Irvine/Tustin to initiate a charge to the account entered for your dues. Charges will show up on your credit card statement as: FitMom Irvine. This authorization is to remain effective until you have completed the cancellation form according to the terms below and have received written confirmation from Fit4Mom Irvine/Tustin that your membership will be cancelled.

You have the right to stop payment on an Electronic Funds Transfer debit by notifying your bank. This, however, does not void your contract with Fit4Mom Irvine/Tustin to fulfill your payment commitment and you are obligated to pay. The processing date for debit cards may vary due to banking procedures. If charges are returned, they will be subject to a late fee. Members are responsible for updating their credit card information. Members may not attend class who have unpaid balances. If a membership goes unpaid for over 30 days, it will be subject to an immediate cancellation.

I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.

I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).

Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.

CANCELLATION AND HOLD POLICY
Membership dues are not attendance-based, and will not be prorated, credited or refunded to due lack of attendance. To place your membership on hold you will be charged a one-time $20 fee to process the hold and maintain your monthly membership rate, no matter the length of the hold period. Membership holds are for a minimum of one (1) month, not to exceed four (4) months each year.

You may cancel or hold your membership by completing the membership status change form online, linked at the bottom of every weekly member email. All cancellations must be received 30 days prior to your next billed date (ie, notify Sept. 1 for Oct. 1 cancellation), or you will still be charged the full dues amount on the first of the month. For all holds, your regular monthly dues will be set to automatically start back up after 4 months. All fees and dues processed are non-refundable. If you are ready to come back to class sooner, you can do so by emailing us and full dues will be prorated less any hold fees already charged. If you decide to cancel during your hold period, you will be charged a $59 early cancellation fee. If your membership is canceled, and you wish to re-enroll at a later date, you will be charged 50% of the registration fee to reactivate your account.

Our instructors cannot authorize hold or cancel requests, either verbally or in writing. Holds or cancellations will only take place for upcoming months. We will not go back and credit dues or fees already paid. It is your responsibility to complete the Membership Status Change Form according to the guidelines and date restrictions set forth. Memberships are not transferable to other Stroller StridesĀ® locations outside of the Fit4Mom Irvine/Tustin franchise or to other persons.

AGREEMENT
By purchasing a Monthly Membership, you are agreeing to the terms of this contract. Terms of this agreement may change with the proper advance written notice.
